Technical Swim Assessment (manual coach form)

A 10-section observation form for coach-filled swim technique notes.

What this assessment is

The Technical Swim Assessment is a manual coach observation form that captures qualitative technique notes from a pool-side or video review. Unlike the EO SwimBETTER upload, this is filled in by the coach by hand and lives as a structured Assessment record.

The 10 sections

The form is broken into 10 sections so it can be filled in over multiple sittings or focused on a single area:

  1. General: preferred distance, weekly mileage, breath hold, open-water and pool confidence ratings (1 to 10).
  2. Technique at a glance: 1 to 5 ratings for body alignment, catch and pull, kick, breathing, symmetry.
  3. Technique: Head & Eyes: eyes position, head position, head stability and axis.
  4. Technique: Entry & Catch: hand entry, entry angle, finger spread / hand shape, wrist angle on catch, catch quality.
  5. Technique: Pull & Recovery: pull path, elbow position through pull.
  6. Technique: Body: rotation, hip drive, kick mechanics, body line.
  7. Starts, turns, open water: race starts, flip turns, sighting, drafting awareness.
  8. Consistency & fatigue: how form degrades with effort and distance.
  9. Physical / mobility: shoulder, ankle, thoracic mobility limiters.
  10. Narrative: free-text coach observations and prescription.

Inputs

Each section has its own fields. Most are choice fields with a fixed option list (e.g. "looking forward / looking down / variable") or 1 to 5 ratings. The narrative section is free text.

How to use it

Use the assessment when you do a pool-side review or video analysis session. Fill in only the sections you observed; the form is partial-save friendly.

Save the assessment when you are done. The record goes into the athlete's assessment history with the date of the review.
